The English version of our registration form is downloadable in Adobe PDF format only. If you wish to place membership with CCSA, please print out the form and fill it out by hand. You may then bring it to our office on the specified days of registration directly or mail it to us. PLEASE DO NOT E-MAIL THIS FORM. E-mailed forms will be ignored and disposed of.
